چکیده

tor of the liver was identified as an aberrant coronary vein ( fig. 1 ). A superior mesenteric arterioportogram revealed portal vein obstruction, resulting in portal blood flowing back to the right gastroepiploic and splenic veins. Venous blood of the lesser curvature then drained into the left portal system via the aberrant coronary vein ( fig. 2 ). Cavernous transformation of the portal vein is a typical finding when the portal vein is obstructed with or without the intrahepatic branch. An aberrant coronary vein can be a hepatopetal collateral pathway when the portal vein is obstructed as in the present case. The coronary vein normally joins the portal vein, the splenic vein, or the confluence of the two. In contrast, persisting anastomoses among the foregut venous plexus or omphalomesenteric veins result in various venous anomalies, including the aberrant coronary vein, which originates from the lesser curvature of the stomach, moves through the lesser omentum and drains directly into the left portal system. Computed tomography of a 50-year-old man revealed a low density mass, 10 cm in diameter, in the retroperitoneal space behind the pancreas head. A thick vein from the lesser curvature that drained into the left lateral secPublished online: September 26, 2008

Bleeding oesophageal varices in the absence of intrahepatic or extrahepatic obstruction of the portal system and without portal hypertension.

OESOPHAGEAL VARICES are generally secondary to portal hypertension and are therefore usually associated with obstruction to the portal circulation within the liver as in cirrhosis or in the portal vein itself. Thus, according to the site of obstruction, portal hypertension is usually classified as intrahepatic or extrahepatic in type (Whipple, 1945). However, an increasing number of cases of po...

Use of meso-Rex shunt with transposition of the coronary vein for the management of extrahepatic portal vein obstruction

The meso-Rex shunt is used to safely and effectively treat patients with portal hypertension due to extrahepatic portal vein obstruction. In the standard meso-Rex shunt technique, the patient's own internal jugular vein is used as a vascular autograft. Inevitably, such a procedure requires neck exploration and sacrifice of the internal jugular vein. Here, we present a case of a 20-year-old man ...

Epiploic gonadal vein as a new bypass route for extrahepatic portal venous obstruction: report of a case

A 61-year-old man was referred to our hospital to treat extrahepatic portal venous obstruction. Endoscopic injection sclerotherapy (EIS) was performed for the esophageal varices; however, the patient returned with massive hematemesis from gastric varices 6 months after treatment. Although the varices were treated with EIS, gastric devascularization and splenectomy concomitant with shunt surgery...

Ultrasonographic evaluation of partially attenuated congenital extrahepatic portosystemic shunts in 14 dogs.

Doppler ultrasonography was used to evaluate the portal vein in 14 dogs before, immediately after and four weeks after a partial ligation of a congenital extrahepatic portocaval shunt. By four weeks after the operation, the hepatofugal or zero flow in the portal vein segment cranial to the shunt origin had become a hepatopetal flow in 13 of the dogs, which became clinically healthy. The other d...

Variant meso-Rex bypass with transposition of abdominal autogenous vein for the management of idiopathic extrahepatic portal vein obstruction: a retrospective observational study

BACKGROUND The aim of this study was to evaluate whether variant meso-Rex bypass with transposition of abdominal autogenous vein can be used as an alternative treatment modality for selected patients with symptomatic extrahepatic portal vein obstruction. METHODS This was a retrospective review of six consecutive patients who received this alternative procedure for the treatment of symptomatic...
